Event Clearance Form
Complete this form in order for event approval. This form should be completed for both in-person and virtual events at least three weeks before your event date. Click here for room and space diagrams to help with your event planning.

Complete this form in order to get approval for a purchase or pay for a service.

Food Form
The Food Form should be filled out to order food for a student organization event or meeting.
Fundraising Form
Complete this form to register your fundraiser. This should be completed for bake sales, item sales, and other fundraising events. Please print and drop off in OSILD or email a copy to your Program Coordinator.
Gift Card Form
Complete this form to request a gift card as a prize for an event or contest. Once completed, attach to your Finance Form to submit to your Program Coordinator.
Hazing Law Acknowledgement Form
Massachusetts State law requires that each organization distributes a copy of the Hazing Law to all members. State law requires a written acknowledgment from each organization stating that all members have received a copy and agree to comply.

Complete this form to request moving funds from one budget line to another.
Off-Campus Trip Contact
Complete this form when you are in the beginning stages of planning a trip. Your trip is not approved until this form is returned to you with confirmation from your Program Coordinator.
Resource Room Request

All-Organization Awards
All nominations for awards for the All Organization Banquet are due by Friday, April 11, 2025.
The Student Organization Hall of Fame Award recognizes the contributions of a dedicated member of a student organization. The Helping Hand Award is dedicated to an advisor, faculty member, or staff person who has shown great support to a student organization.
Outstanding Student Leader Award
This award is given to an outstanding student leader who has made a significant contribution to student life here at Worcester State. This student leader has exceeded all expectations in leadership, engagement, and campus citizenship.
