Documentation Guidelines for AD(H)D
A current and comprehensive report completed by a qualified professional is required and must include the following components:
· AD(H)D Profile: an AD(H)D diagnosis based on DSM-IV diagnostic criteria with evidence of a substantial limitation to a major life activity. Standard scores from appropriate rating scales must be provided.
· Diagnostic Interview: relevant information pertaining to the student’s academic, developmental, medical, psychological, and family history or background and a description of the presenting concern(s).
· Current Academic Impact
Ø Results from a psycho-educational evaluation including cognitive testing, academic achievement testing, and information processing measures. Subtest and standard scores, and percentile ranks must be provided.
· Summary and Recommendations: a comprehensive interpretive summary synthesizing the evaluator’s judgment for the diagnosis and a rationale for accommodations based upon substantial limitation.
